Microsoft recently reported Windows Updates have been causing BSOD on numbers of Windows 10 computers. The related updates are: KB5000802, KB5000808, KB5000809, and KB5000822 and are related to Kyocera and Ricoh printing drivers.

The BSOD looks like this:

Observe the bottom of the BSOD and make sure you see the following:

  • Stop code: APC_INDEX_MISMATCH

  • What failed: win32kfull.sys

Microsoft has released a procedure to fix this issue:

  1. Restart the computer and log back in

    1. To restart the computer from a BSOD, just power off the computer and power it back on again using the power button

  2. Do not start using the computer right away

  3. Run Windows Update (here is a guide on how to run Windows Update)

  4. Do not use the computer during this time

  5. Restart the computer when prompted by Windows Update and the issue should be resolved

If this does not address your issue, please submit a ticket to [email protected] with a screenshot of the BSOD or begin a chat session with our support team.

Identity Management systems will be undergoing maintenance from 7:00 PM to approximately 9:00 PM on Thursday, March 4.

While most redundant applications are expected to continue to operate normally, there is the possibility of brief outages.

These systems include all the IT Division's authentication services, including Single Sign-On, Phonebook, RADIUS (Employee Wireless and VPN), and Multifactor Authentication services.

The Grouper group and role management application will be down for the entirety of this maintenance, along with two of its dependent services, MFA Management (the MFA token management application) and MFA Bypass.

Hold on! Before you rush to upgrade, here are the things to do:

Once you go through the list and decide to upgrade

On Thursday, February 11, 2020 at 6PM, Commons was offline for ~15 minutes while IT performed scheduled maintenance.

On February 24, 2021, smtp.lbl.gov was reconfigured as a CNAME alias that uses Google's SMTP relay service (smtp-relay.gmail.com) instead of being an on-the-hill host (131.243.228.39).

Any on-the-hill systems (ie having an LBL Class B or NERSC IP address) will still be allowed to relay unauthenticated via smtp.lbl.gov and do not need to change their outgoing/outbound SMTP settings.

This change will only be an issue for hosts that do NOT have a route to the Internet, due to either their networking configuration and/or firewall rules, or can only connect to an IP address, as opposed to a hostname.

More information about the SMTP relaying reconfiguration plan is at:
https://go.lbl.gov/smtpRelayChange
