All Operations personnel will follow the procedure below. Scientific divisions should check with their divisions for proper instructions. IT Support Services will be glad to help Scientific divisions with shipping out boxes if requested.

  1. Send an email to [email protected] requesting you wish to return computer equipment and peripherals and a help ticket will be created.
  2. Enter in the subject line: Please provide a shipping box and a return shipping label for (Employee name & Employee number) 
  3. Provide the information below in the body of the e-mail message:
    1. Full name of the person
    2. Mailing address
    3. Mobile phone number
    4. DOE Property Barcode number of asset
    5. List of items to be returned (including peripherals)
    6. Project-Activity ID

Note:  IT can also process the return of an LBNL ID badge and parking permit.

The IT Support Services team will send you postage paid return boxes for your equipment.
