Requests for service must be submitted by a designated [Telephone Coordinator|Telephone Coordinators]. Division level written approval is required to be added to the list of Telephone Coordinators. For further information, please call the Telephone Service Center at x7997 or send email
*General Information :*
The Telephone Service Center handles all requests for cellular service and equipment. The cost of equipment
may vary, depending on the type being requested.
All cellular requests must have division level authorization (Division Director, Deputy Division Director,
or Division Director's Designate). In addition, as Laboratory policy states, all new cellular users
must read and sign the [cellular phone procedures|http://tscweb.lbl.gov/cellular/cellular.guidelines.html].

1.*To TheOrder following servicesCellular Service:*
You may besubmit your obtainedrequest by [sending e-mail to [[email protected]|mailto:TSC@lbltsc@lbl.gov], or by sending a memo to the Telephone Service Center , mailstop 50E0101.
*Please provide Changethe userfollowing nameinformation:
* Order headsets, pagers, calling cards, or cellular equipment
* Rent conference phones
* Install data jacks (include floor plan as attachment)
* Make any of the following modifications to existing telephone equipment:
** Change number of rings
** Change calling level of access
** Add voice mail to existing extensions
For any of the above services, sendUser's Name
* Type of equipment
* Purchase project ID
* Monthly project ID
* Proper approval signature (division level authorization is required)
*Calling Plans:*
Unless a calling plan is requested at time of cellular service activation, new cellular service subscribers are
placed on the basic rate plan. Changing to or from a calling plan requires the approval of the Division Manager,
Duputy Division Manager, or Division Director's Designate, and is based on business need. For detailed information on available calling plans, [click here|http://tscweb.lbl.gov/cellular/callingplans.html].
*Cellular Trouble Reporting:*
For assistance with minor cellular troubles such as:
* resetting your voicemail password
* setting the phone to ring/unmuting
* setting up the ring option
* setting up voicemail
* activating call waiting
* dropped calls
please call the AT&T Mobility National Care Desk at 800-999-5445 or dial 611 on your cellular phone.
*Note:* If you are experiencing problems with coverage or with the quality of your cellular service, please send email to the [Telephone Service Center at [[email protected]|mailto:[email protected]] and be sure to include your service charge project ID with your request.
2. The following services may be obtained by filling out a [Telephone Services Order Form|http://tscweb.lbl.gov/request/entry.html] on the Web.
* Install new telephones and auxiliary equipment
* Move, change features, extensions or set types of existing equipment
* Disconnect telephones and auxiliary equipment
or call extension 7997.
*Lost or Stolen Cellular Phone{*}* *
Report cellular phones lost or stolen on-site to Protective Services at extension 5472. Report the theft of cellular phones stolen off site to the nearest police department. Request a copy of the police report. Notify the Telephone Service Center at extension 7997. Copies of the police report should be forwarded to the Cellular vendor as well as the Telephone Service Center. The Telephone Service Center cellular representative will notify the cellular vendor and request deactivation of the service, so that further usage is discontinued. New purchase charges are applicable for replacing lost or stolen cellular phones.
